It takes forever to die from diabetes even if you don't take care of yourself. He'll lose a foot, develop really bad eyesight and die of renal failure when he's old.

Doesn't his condition make it really easy to kill himself? Like, isn't it a simple matter of starving himself for 24 hours or just injecting too much insulin?

Bob is not simply a fat fuck, he's a miserable fat fuck. The distinction is an important one, particularly with regards to how his health is going to deteriorate.
